Media Consent

By submitting/signing this form, you give permission for your image, video, or audio recording to be captured and used by organisations delivering or supporting the Resilient CARE program. This includes use in program-related communications such as websites, social media, newsletters, printed materials, reports, presentations, media, and public relations.

This consent applies to any Resilient CARE program partner or funder involved in program delivery, including (but not limited to) community organisations, health service providers, government agencies, and contractors.

You understand that:

  • Your participation is voluntary and you can withdraw or update this consent at any time by contacting the organisation who collected it.
  • Your image or recording may be edited and used in part or full.
  • It may be used for current and future communications related to the Resilient CARE program, unless otherwise stated.
  • You will not receive payment for the use of your image or voice.
  • Materials already published may not be withdrawn from public circulation.
  • Your image and story will be handled respectfully and, where applicable, with cultural sensitivity.

Privacy Notice: Your personal information will be collected and stored by the organisation administering this form (Country to Coast QLD) and used only for purposes related to the Resilient CARE program. It will be managed in line with that organisation’s privacy statement. You may request to access or correct your information, or withdraw consent, at any time.
